Make a medicine list to discuss with your pharmacist.
Add the traditional medicines and other medications you currently use, plus optional health context such as age, medical conditions, pregnancy or breastfeeding. PharmAlign does not show interaction results or treatment suggestions on this patient form. Your pharmacy can review the encrypted information in PharmAlign and continue the discussion with you.
Add a traditional medicine
Search by traditional/common name, publicly documented Indigenous-language name, alternate name or scientific name. The name you select is carried into the pharmacist review. The full directory stays collapsed unless you choose to browse it.
Traditional names and spellings can vary by Nation, community and dialect. The list is intended to help identify what you use so your pharmacist can review it with you.
Add prescription and over-the-counter medicines
Search by generic name or common Canadian product/brand name. Every medication can still be added exactly as entered if it is not in the indexed list; manual entries are flagged for pharmacist verification and are not treated as automatically interaction-checked.
Add health context (optional)
This can help your pharmacist recognize patient-specific cautions and areas that may need closer monitoring. It does not generate advice for you, and your pharmacist must verify the information and assess it in context.
Medical conditions
Search common conditions or type another condition exactly as you know it. You can add more than one.
Continue the discussion with your pharmacist
Traditional medicines, other medications and patient-specific health factors can affect what needs to be reviewed or monitored. This patient form does not display interaction information or recommend changes. When your list is ready, send it to your pharmacy so a pharmacist with PharmAlign access can independently assess it with you.
